چکیده
هدف: مدل شفایافتهٔ آمیخته نوع خاصی از مدل‌های بقا است و در آن فرض می‌شود که جامعه مورد بررسی آمیخته‌ای از افراد مستعد برای تجربهٔ رویداد مورد نظر و شفایافتگان (افراد نامستعد) که هرگز رویداد مورد نظر را تجربه نمی کنند، می‌باشد. برای چنین داده‌هایی مدل‌های استاندارد بقا معمولاً مناسب نیستند زیرا در آن‌ها امکان شفایافتگی در نظر گرفته نمی‌شود. هدف این مطالعه، بررسی تأثیر عواملی مانند سن، دیابت، صدمه به سر و گردن و ... بر شفایافتگی یا فوت و سپس برآورد نرخ این دو شاخص در سالمندان دارای صدمات ناشی از سقوط است. بدین‌منظور پس از سقوط سالمند و مراجعهٔ وی به بیمارستان، عوامل فوق اندازه‌گیری شده و پس از پیگیری وضعیت (شفایافتگی یا مرگ)، میزان شفایافتگی و فوت محاسبه شده است؛ به‌عنوان مثال نرخ فوت در سالمندانی که دارای بیماری دیابت هستند با افرادی که دارای این بیماری نیستند، مقایسه شده است. 
روش‌بررسی: در این مطالعه از داده‌های تمامی سالمندانی که به‌علت سقوط صدمه دیده بودند و پروندهٔ پزشکی آن‌ها طی سال‌های ۱۳۸۹ تا ۱۳۹۱در بیمارستان شهرستان قم موجود بود، استفاده شد. در ادامه، با پیگیری از ادارهٔ ثبت‌احوال، وضعیت مرگ یا زندگی آن‌ها ثبت شد. تا اردیبهشت‌ماه ۹۴ (به فاصلهٔ زمانی حداقل سه‌سال و حداکثر شش‌سال پس از سقوط)، در کل؛ ۱۲۴مورد برای تحلیل، استفاده شد. برای تحلیل از مدل‌های شفایافتگی آمیختۀ نیمه‌پارامتری خطر متناسب و مدل‌های شفایافتگی آمیختهٔ شتابی زمان شکست برای داده‌های سالخوردگان دارای صدمات ناشی از سقوط استفاده شد و نتایج مقایسه گردید. برای تجزیه‌وتحلیل داده‌ها از نرم افزار R نسخهٔ ۳-۲-۳ استفاده شد. 
یافته‌ها: از نمونه‌های بررسی شده، ۵۴نفر از سالمندان، مرد (۴۴درصد) و ۷۰ نفر از آن‌ها زن (۵۶درصد) بودند. تعداد ۲۰نفر (۱۶درصد) دارای سابقۀ بیماری قند و ۳۹نفر (۳۱درصد) دارای پُرفشاری خون و تعداد ۶۴نفر (۵۲درصد) دارای صدمه به سروگردن و تعداد ۴۸نفر (۴۳درصد) دارای یک موضع جراحی‌شده بودند. برازش مدل چندمتغیره برای شفایافتگی آمیختۀ زمان شکست شتابی نیمه‌پارامتری نشان داد متغیر سن بر احتمال شفایافتگی تأثیرگذار است (۰٫۰۱۶=p، ۰٫۰۳۹-=b age) و سایر متغیرها بر شفایافتگی تأثیری ندارند. برای فوت‌شدگان، متغیر صدمه به سروگردن (۰٫۰۳۱=p و ۰٫۱۶۵=b head neck)  و متغیر دیابتی‌بودن (۰٫۰۸۰=p و ۰٫۱۵۵=b head neck)  بر احتمال فوت تأثیرگذار بود.
نتیجه‌گیری: با توجه به اینکه صدمه به سروگردن یکی از عوامل مؤثر بر فوت سالخوردگان دارای صدمات ناشی از سقوط بوده است، به نظر می‌رسد با بهبود شرایط مکان زندگی سالمند، می‌توان آسیب‌های ناشی از سقوط را کاهش داد. علاوه‌بر این، با کنترل تغذیۀ سالمندان می‌توان آسیب‌های ناشی از بیماری‌هایی مانند قند را به حداقل رساند. هم‌چنین هر دو روش شفایافتگی آمیختهٔ خطر متناسب و روش زمان شکست شتابیده قادر به برآورد پارامترهای مدل شفایافتۀ آمیختۀ نیمه‌پارمترای داده‌های سالخوردگان ناشی از سقوط است.
