Background & Objective: Stuttering is an interruption in the flow of speech that is associated with certain types of inconsistencies, such as repetition of sounds, syllables, and monosyllabic words. These inconsistencies can affect the speed and rhythm of speech and may have negative reactions. Be associated with talking, avoidance behaviors and physical tension. Transcranial Direct Current Stimulation is a non-invasive method of brain stimulation that may lead to improvement of speech disorders in people who stutter. tDCS involves applying a weak electrical current across the head through electrodes placed on the scalp, modulating the membrane potential of neurons in the underlying cortex. The aim of this study was to investigate the effect of anodal transcranial Direct Current Stimulation of left frontal lobe on social anxiety and stuttering severity of adults with stuttering disorder.
Methods: In this semi-experimental study with pre-test and post-test, among stuttering patients referred to the neurology and rehabilitation clinic in Yazd city, 30 people in the age range of 20 to 30 years were selected, 15 of them. They were in the intervention group and 15 people were in the control group. The scores of the social anxiety variable by Connor's social anxiety test (2000) and the stuttering severity variable by the SSI-3 stuttering severity test (Farazi, 2013) were compared at two time points before and after the electrical stimulation treatment. In the control group, each patient received 10 sessions of repetitive artificial magnetic stimulation of the brain from the skull (20 minutes) and in the experimental group, 10 sessions of anodal tDCS (20 minutes) with a current of 2 milliamps in the left frontal cortex. Data analysis was done using covariance analysis test. The significance level of the tests was 0.05.
Results:  Data analysis was done using covariance analysis test.Data analysis showed that the score of social anxiety test and severity of stuttering in stuttering patients in the intervention group and after receiving treatment has decreased significantly compared to the control group respectively (p-value=0.001).
Conclusion: The left hemisphere of the brain is responsible for positive moods and emotions, and the right hemisphere is responsible for negative moods and emotions. Anodic tDCS on the left frontal cortex can stimulate positive mood and emotions in stuttering patients by increasing the level of neuronal excitability in the left cingulate, thus improving the social anxiety of patients with stuttering disorder, and subsequently stuttering also decreases. According to the results of this research, it seems that the anodal tDCS of the left frontal lobe along with progressive muscle relaxation exercises improves social anxiety and stuttering severity in adult stuttering patients, and these methods can be used along with other methods. He used a treatment to rehabilitate stuttering patients.To explain the results of this research, it can be said that patients who undergo a frontal lobe lesion often have a transient speech disorder after surgery, which may be related to the removal of the supplementary motor area. Postoperative neurological impairment is characterized by a decrease in spontaneous speech and a significant decrease in speech output, including stuttering. Functional MRI (fMRI) showed that the most important brain area for speech fluency is the frontal lobe. tDCS in the area of the left frontal lobe can lead to a lasting improvement in the speech fluency of patients with dyslexia by modulating the neuronal function and the level of neuronal excitability.
